Application for people with mobility restrictions

Step 1. Download the current application form. Two variants are available:

Step 2. Complete Section 1 of the form and (optional) prepare copies of recent medical reports.

Step 3. Take your form to your Doctor or Occupational Therapist to complete Section 2.

Step 4. Submit the completed application form and report copies using one of the following options:

  • Email: acrod@nds.org.au (make sure to include all pages and medical reports)
  • Online: acrod.org.au/apply-mobility (you’ll need to scan or take clear photos and upload all pages of the form and reports)
  • By post: ACROD Parking Program, PO Box 184, Northbridge WA 6865

Application for people who are legally blind

Alternatively, take the following steps to fill out the application form manually.

Step 1. Download the current application form. Three variants are available:

Step 3. Attach ONE of the following documents:

    • Disability Support Pension – Blind card (front and back);
    • Aged Pension – Blind card (front and back);
    • Department of Veteran Affairs’ Blind Concession Card;
    • Guide Dogs WA Dog Handler Card;
    • Vision Australia Seeing Eye Dog ID Card;
    • ACROD Low vision certificate form completed and signed by a specialist or a medical practitioner. You can download the ACROD Low Vision certificate form in .PDF format here;
    • A report from a specialist or a medical practitioner (for example, an ophthalmologist, orthoptist, optometrist or GP) addressing the eligibility criteria and confirming legally blind status.

Step 4. Submit the completed application form and the attached document using one of the following options:

    • Email: acrod@nds.org.au; or
    • By post: ACROD Parking Program, PO Box 184, Northbridge WA 6865

Assessment of Applications

The ACROD Parking Program assessment team will assess each application against the eligibility criteria. If we need more information, the ACROD Parking Program team may:

  • Contact the applicant (or legal guardian) to ask for additional information; or
  • Contact the specialist or medical practitioner who signed the application form or attached documents to ask for additional information.

Depending on the applicant’s circumstances, we may issue a renewable or a temporary ACROD Parking Permit.

Renewable permits are issued to applicants who meet the eligibility criteria, and their condition is expected to last for the rest of their life, despite treatment and therapy options. These permits are renewed every 2 years.

Temporary permits are issued to applicants who meet the eligibility criteria, and their condition is expected to improve or may improve in the future. Temporary permits are issued for 6 or 12 months and can be extended once.

The ACROD Parking Program receives a high number of applications. Processing your application may take up to 20 working days from the date your application is received. This does not include mailing time.

If your application is approved, we will send you your ACROD Parking Permit by post. If your application is not successful, we will provide information about our appeals process.
